This explains what data is being collected and how your information is being used.
What data is collected and how is the information used?
- Name and Age – Basic information to help establish the client and for the practitioner/therapist to get to know the client.
- Address, Email Address, Phone Number – This is used to contact the client regarding sessions or topics discussed in a session.
- Emergency Contact/Medical Professional Details – If a concern arose about the client potentially being at risk of danger or harm, the practitioner/therapist will contact emergency contact and/or medical professional. If an immediate risk of danger or harm arose, the practitioner/therapist will then contact the appropriate governing body or organization.
- Session Notes – Brief notes of each session are kept in a client file to maintain all information. These files and notes are locked in a lockable filing cabinet for client privacy at the end of each day.
Will data be shared and if so, who will it be shared with, and for what purpose?
It is very unlikely that the data will be shared, but there may be instances where the data will need to be shared.
NOTE: The data will not be sold or used for unethical or unauthorized purposes.
- Court/Law Enforcement – If in-session discussions are had about the client or someone associated with the client that is at harm or at risk of harm. If the notes and files of the client are subpoenaed.
- Governing Body/Organization – If in-session discussions are had about the client or someone associated with the client that is at harm or at risk of harm.
How will data be stored?
Hard copies are stored in a locked filing cabinet after the workday is completed. During the day it will be in the filing cabinet unless in a session.
Digital copies will be stored in a password-protected computer. At the end of the business day, the computer will be shut down for the day.
How long will the data be stored?
The data will be kept for the time required by law, which is currently 7 years.
How will the data be disposed of at the end of its storage period?
After the time required by law of 7 years, all hard copies of the data will be shredded by a diamond cut/cross cut shredder.
